    Overview

    Child(ren) will decorate a person that resembles themselves; practice and learn their phone number, address, age and birthdate. 

    Summary

    Create a personal information sheet.  Allow your child(ren) to decorate a person that resembles themselves, practice and learn their phone number, address, age and birthdate. 

    Background for Teachers/Parents

    Be aware of your child(ren) and their home situation when it comes to phone numbers and addresses. Use sensitivity as needed.

    Consider the support your child(ren) may need.  For example, they may need the information added to a sticker label for them to add to the sheet, they may need you to write with a highlighter that they can trace, they may need to have a paper to copy the information from.

    Intended Learning Outcomes

    Children will be able to tell you at least their name and age.  More practice may be needed for the phone number and address.
